How to access the service
People who have diabetes
You can phone 1300 566 899 and ask for a time to see a Diabetes Nurse Educator. To make the most of your appointment bring with you:
- List of medications
- Any information about your diabetes treatment
- Any blood glucose monitors and record books
- You may like to ask your GP for a referral letter, but this is not necessary
Health professionals
Please fax a referral letter to (03)6431 8061 for Burnie-based referrals or (03)6421 7767 for Devonport-based referrals. Referral letters can be posted to Tasmanian Health Organisation — North West Diabetes Centre: PO Box 258, Burnie TAS 7320. Please include in referral:
- Patient details
- Health summary
- List of medications
- Relevant pathology
- Anything else of relevance

General referral

For insulin management
If you wish your patient to have any insulin doses titrated by the Diabetes Educator on your behalf, you will need to complete and sign a pro-forma referral form. The form can be faxed to Burnie - (03) 6431 8061 or Devonport - (03) 6421 7767.
For Insulin commencement
If you require your patient to talk with a Diabetes Educator about starting insulin in order to highlight your concerns or to reiterate your management plans - you can follow ordinary referral pathway.
If the decision has been made for your patient to attend for initiation of insulin immediately you will need to have:
- A script filled for the patient, who will be required to bring their insulin with them to the appointment.
- In addition to the ordinary referral pathway, the Diabetes Educator requires permission to initiate insulin therapy in the form of a pro-forma referral form. This needs to be signed by you and faxed to either Burnie - (03) 6431 8061 or Devonport - (03) 6421 7767.
